Coverage Details
- Associate coverage: $10,000–$250,000 in $10,000 increments
- Spouse coverage: $5,000–$125,000
- Child coverage: $1,000–$10,000
Eligibility
Full-time associates working 30 or more hours per week
60-day waiting period
30+ hours/week required
